We are looking for an enthusiastic, highly motivated and well-organised Band 5 nurse or therapist to work in the Referral Management Hub in Brighton. The Hub supports all the referrals to Brighton and Hove Responsive Services.

Main duties of the job

The post holder will be required to work as part of a team, but also independently, prioritising referrals using clinical decision-making skills, and seeking support from senior staff to manage complex referrals as required.

The post holder will ideally have an understanding of the services available across our Trust, and be able to effectively communicate with a wide range of people including clinicians, therapists, social care workers, managers, staff, and patients using the services.

Detailed job description and main responsibilities
The role involves the triage and coordination of referrals that support early hospital discharges and avoidable hospital admission. Referrals come from hospitals, GPs, paramedics and other agencies.

Qualifications
Essential criteria

  • RN level 1 or Allied Health Professional with HCPC Registration
  • Evidence of training and development undertaken during current employment or recent employment
  • Desirable criteria

  • Mentorship Preparation Module or equivalent
  • Experience
    Essential criteria

  • Two years experience within the NHS or social care setting post qualification
  • Experience of working in a multi-disciplinary team
  • Desirable criteria

  • Experience of using multiple methods to collect and record data to support service delivery
