An exciting opportunity has arisen to join our Learning Disability service . An opening has arisen for an experienced q ualified Social Worker to join our South East Duty team, which is based in Havant, although the Team are home working currently. This position is for 12 months, due to the current post holder having an opportunity to develop their career.
As a qualified Social Worker , registered with SWE , you will need strong interpersonal and organisational skills. In this role you will complete short term work, responding to immediate safeguarding concerns or risks, whilst liaising with the Long term team, if a situation requires longer term involvement. We support people with learning disabilities to gain greater independence to meet their outcomes. You will need to travel independently to meet with individuals and carers to assess needs, to undertake strength based reviews using a person-centred approach designed to provide meaningful choice and promote wellbeing.
Working in accordance with social care legislation and local government requirements, informed by the values and principles of social care; you will be supporting in the delivery of person centred, safe and effective social care service demonstrating excellent strength-based practice.
You will have excellent case management skills or relevant transferable skills, as well as risk assessment skills. The ability to work in partnership with individuals to identify their care and support needs is vital to the role and will draw upon your excellent communication skills.

An excellent working knowledge of legislation and current policies is required. Social Workers in the team are required to undertake various duties to include assessments under the Care Act 2014, mental capacity assessments, reviews of funded care and safeguarding work.
Due to the nature of the role, the successful candidates will have a full driving license and access to a car.
