An exciting opportunity to join our Hospital based team.

Hampshire County Council are looking for a Senior Practitioner to join our friendly and supportive hospital team based at the Queen Alexandra (QA) Hospital in Portsmouth. Focussing on providing support for older persons and adults with physical disabilities. In the current climate the team are working both to discharge people from hospital (QA and community hospitals) but also to support a discharge to assess (D2A) model, so full assessments are completed post discharge to determine an individual’s longer term/ onward care plans. 

The newly opened Clarence unit supports with the discharge to assess approach and the hospital team also have dedicated link workers supporting the onward care planning for individuals transferred to this unit. 

In the role you will undertake proportionate assessments with service users and their families to deliver safe and timely hospital discharges which offer our residents the best opportunity to maximise their independence and wellbeing. Committed to collaborative multi-disciplinary working with partners and provider services, you will problem solve and promote person centred care planning. You will be committed to a discharge to assess approach and therefore support more fuller care act assessments post discharge. 

About you:

As a highly motivated individual, with drive and commitment, you will enjoy a fast-paced environment and be committed to working in a strengths-based manner to support Hampshire residents. You will have an understanding of social care provision/relevant previous experience and be willing to further develop your knowledge. 

You will play a pivotal role in working as an intermediary between service users, their families and service providers, managing a mixed caseload and be flexible in approach and enjoy team working. 

Being a fully qualified Social Worker and HCPCregistered, we are looking for experience in areas including Safeguarding, complex case work, resource allocation and funding. You will be joining a very experienced and accomplished team of practitioners and a supportive management ensuring you transition into the team efficiently and smoothly. 

The post has a requirement to work at weekends on a rota basis.
