The Role:

We have an exciting opportunity within the Adults’ Health & Care Reablement Service, to welcome a new enthusiastic, self-motivated, and professionally qualified Senior Practitioner into the team.

The Reablement service consists of a multi-disciplinary team of Occupational Therapy, Sensory, Bed Link Worker and registered care delivery staff, working together to achieve the best quality outcomes for individuals. This post will focus on providing great leadership across the Reablement service, but more specifically to the Sensory cohort of staff within the team. You will ensure effective management, coordination and allocation of case work and triage amongst the team and robust effective processes for workload management. You will be involved in change management and service development activity including providing an integral contribution to a Rapid Improvement Event in train on a countywide basis currently, alongside sensory leads within other Reablement hubs.

Additionally, you will support the wider Reablement Service and system, including providing a cross-discipline triage lead function. Alongside your peers within the leadership team, you will contribute to the overall effectiveness of the Single Point of Access (SPoA), contributing to and chairing robust multi-disciplinary discussions, ensuring safe hospital discharges into Reablement and the Rapid Discharge Service. You will have a strong commitment to achieving quality outcomes for individuals, to working with a strengths-based and positive risk assessment approach. You will have excellent communication skills, will be comfortable working at pace and able to work well under pressure.

As a Senior Practitioner you will line manage staff and be committed to their wellbeing and development. You will join and be supported by a strong and established leadership team who share a commitment to delivering an exceptional reablement service.

The reablement service operates 7 days a week and there will be a requirement to work some weekends, on a rotational basis.
