We are seeking to recruit skilled and experienced Senior Community Reablement Officer to work within a service offering short term programmes of bespoke Reablement interventions. We are looking for people who have a positive attitude, are innovative, and enthusiastic. Service delivery is based on needs assessment; is person centred and outcomes focused; which supports the health, wellbeing and independence of customers identified as having Reablement needs. This service contributes to the Borough wide delivery of Reablement services which is part of the Croydon’s Living Independently For Everyone, (LIFE), project.
The service operates seven days weekly, covering the hours between 8am and 8pm and you will be required to work on a rota system.
You will have knowledge, skills and experience relating to and being part of team, and be able to work on your own. Facilitating and implementation of agreed personal support plans to Reablement Outcomes.
Providing agreed personal care as per agreed Reablement Plan and outcomes in order to support individual clients in achieving their agreed Reablement outcomes by working with them and carers in their own home including monitoring and reviewing of Reablement outcomes or signposting them to community universal activities of choice, by advocating on their behalf with partner organisations.
You will have experience of the application of Confidentiality and Data Protection protocols, report writing, case recording, and of working with colleagues from a range of other statutory and non-statutory organisations.
You will also need to show an understanding of reablement and community networking to support customers to maintain independence, choice, wellbeing and their potential use of universal services.
Applicants are expected to have experience of working with vulnerable customers within a Safeguarding framework and have an understanding of relevant legislation.
All applicants should hold a current driving license as you will be required to use your own vehicle to visit customers.
