We are seeking an enthusiastic and highly motivated Clinical or Counselling Psychologist, who is passionate about improving the lives of people with a learning disability in North Derbyshire.
The role involves supporting individuals and their families or carers with high quality psychological assessments, formulations and interventions. You will be working in the community in a range of settings, as part of a supportive multi-disciplinary team.

The caseload will be interesting, varied and include working with people with a wide range of needs.
You will join a diverse group of psychological practitioners and a service committed to on-going professional development for staff.

The post is based in north Derbyshire, close to the beautiful countryside of the Peak District National Park, with good transport links.
Main duties of the job
Provide a key clinical role as a member of a Community Learning Disability Team who work with the individual in their community to provide responsive, specialist psychological care for to maintain the individuals engagement with their home, family and community.
This involves taking a key role in developing, implementing and evaluating assessments, formulation and treatment plans in relation to their mental wellbeing, offering clinical advice and strategies regarding risk management.
Provide specialist consultation, assessment, formulation, intervention and education of psychological and emotional difficulties / presenting problems, for people with a learning disability.
Undertake partnership working with Intensive Support teams, Inpatient teams, Adult Care and other key partnerships. This will be a vital element to provide holistic person centred care.

Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.

Detailed job description and main responsibilities
From 11 November 2021, everyone working in care homes has to be fully vaccinated against Covid-19 by law to protect residents. As a provider of services to CQC registered care homes, we shall only employ you if you are fully vaccinated in roles that require you to work in a DCHS-led core unit or care home setting (unless you are medically exempt). Mandatory vaccination will be part of your terms and conditions of employment (unless an exemption applies)
Be professionally and legally responsible and accountable for all aspects of own work including clinical input, effective use of time,
prioritisation, punctuality etc.
Provision of specialist psychological assessments of clients referred to the team based upon the appropriate use, interpretation and integration of data from a variety of sources including psychological tests, self-report measures, rating scales, direct and indirect structured observations and semi-structured interviews with clients, family members and others involved in the client’s care.
Formulating and implementing plans for the formal psychological treatment and/or management of a client’s difficulties, based upon an appropriate conceptual framework of the client’s problems, and employing methods based upon evidence of efficacy, across a range of care settings.
Implementing a range of psychological interventions identified from prior psychological assessments for individuals, carers, families
and groups, adjusting and refining psychological formulations as required.
Evaluating and making decisions about treatment options taking into account both theoretical and therapeutic models and highly complex factors concerning historical and developmental processes that have shaped the individual, family or group.
Formulate discharge summaries and psychological reports that reflect reason for referral, assessment, intervention, outcomes and recommendations.
Exercising autonomous professional responsibility, within the context of multidisciplinary team working, for the assessment, treatment and discharge of clients.
Consulting and advising, colleagues from other disciplines and services on the psychological formulation and management of clients with learning disabilities, and in respect of areas of specialist knowledge.
Contributing directly and indirectly to a psychologically based framework of understanding and care to the benefit of all clients of the service, across all settings and agencies serving the client group.
Maintain accurate, comprehensive and timely records in line with professional, legal and Trust requirements. To comply with
statistical requirements for the Department and Trust.
Observe the codes of conduct and professional standards of The Health and Care Professions Council and the British Psychological Society.
Be aware of Clinical Governance issues across the service, including health and safety, risk management, audit, evidence-based
practice etc. and contribute to this process.
Evaluate current practise through the use of outcome measures, evidence-based practice projects, audit and research.
Be involved in professional activities within the Practitioner Psychology profession and outside the profession e.g. multi-disciplinary
forums or networks in order to share practice and promote Psychology services to other agencies.
Respond to individuals in crisis in the community ensuring that an appropriate aftercare is actioned.
Ensure that patients receive a flexible proactive response during crisis episodes in line with current pathways and access criteria.
To undertake relevant clinical assessments and advise on the management of risk behaviours.
Support individuals and their carers to be included and empowered during service delivery.
Provide a person centred approach that meets the individual needs of patients accessing the service.
Ensure that clients receive a confidential service which provides privacy, dignity and respect at all times.
To work in partnership with inpatient areas to aid with care and support either pre-admission or discharge.
Comply with all DCHS policies and procedures.
