Main duties of the job

The post holders will be expected to work together to:

  • Deliver local support to patients diagnosed with Huntingtons Disease (HD) in North Wales as a key worker.
  • Track patients along their disease pathway from diagnosis to end of life care.
  • Coordinate the local multidisciplinary care of patients with HD across North Wales in primary and secondary and Third Sector care ensuring patients can access high quality care that adheres to the best standards of care and best practice guidance.
  • Liaise with the the Huntingtons Disease tertiary care clinic for advice
  • To be a single point of contact for patients with HD, their carers and professionals as a resource for information advice and signposting.
  • Co-ordinate ENROLL-HD research project in regards to recruitment and retention. 
  • The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

    Essential criteria

  • Registration with professional body(nursing, relevant therapy body e.g. Physiotherapy , Speech and Language Therapy Occupational Therapy)
  • Relevant 1st degree or evidence / working towards, supplemented by diploma level specialist training or experience
  • Experience

  • Significant post registration experience including experience relevant to palliative care and community working
  • Broad range of community experience of speciality
  • Experience of working in a multidisciplinary team
  • Demonstrate leadership skills and experience
