Main duties of the job

The post holder will provide clinical leadership within the Community setting including care homes and GP Practices. The post holder will work closely with the multi-disciplinary team and other community services and be able to practice at an advanced level of professional autonomy as well as being a good team player.

The role of Trainee Advanced Nurse Practitioner is key to driving standards for professionals and ensuring the delivery of care to patients is exemplary and timely within a proactive community hospital.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
