We believe in developmental and training opportunities for our staff and therefore are currently seeking to recruit highly motivated Band 5 Community Staff Nurse to support in the delivery of the Healthy Child Wales Programme.

The successful applicant will need to plan, implement and evaluate programmes of care for children and their families within a community setting. Adjust plan or intervention according to changing healthcare needs of children, young people and their families.

To work collaboratively with other professionals ensuring the child/familys needs are met, in relation to ongoing care needs, risk assessment and management, child protection procedures and transition to other services. 

You must be able to travel between patients homes both within towns and rural areas in a timely manner.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
