Main duties of the job

As Care Coordinator, you will hold a caseload of patients with organic and functional mental health needs, travelling to see them in their own homes, in care homes and in-patient settings.

Be part of the Duty Nurse rota, covering both North and South Denbighshire for our service- undertaking first line mental health assessments, risk assessments and supporting Mental Health Act assessments as required

Undertake supervision of Band 5 CPNs, HCSWs and Student Nurses as required

Administering of Depot medication as required

Autonomous and joint working as part of a Multi Disciplinary team

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
