Main duties of the job

Demonstrate leadership through clinical expertise delivering high standards of personalised care by acting as a key worker, identifying needs through holistic needs assessment and co-creating a 
care and support plan for women with cancer.

To undertake holistic needs assessment, care planning and evaluation from diagnosis, through treatment and beyond.

To establish working relationships and communication links between primary secondary tertiary and independent care providers to improve care for patients and provide specialist clinical 
expertise, advice and support when required.

To be the patient advocate within the site-specific Multi-disciplinary Team (MDT). 

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
