ACRT is an interdisciplinary community rehabilitation team with 2 specialist pathways for clients living with physical or neurological long term conditions. The team is made up of Physiotherapists Occupational Therapists, Speech & Language Therapists, Community Dietitians, Clinical Psychologists, a Neuropsychologist, Rehabilitation Assistants and Administrators. The team is supported by a Consultant Geriatrician, Consultant Neurologist and an AHP Consultant.

This post is suitable for a highly motivated, experienced Occupational Therapist, Physiotherapist, Speech and Language Therapist or Dietitian interested in developing their leadership and management skills with proven experience working in the community rehab setting.

You will work closely with the Head of Service taking forward clinical and service developments for ACRT. The integrated care agenda and the development of Neighbourhoods across City & Hackney brings about the opportunity for project work within this role.

You will be an enthusiastic practitioner with excellent communication, organisational and leadership skills. You will take a lead on operational management for ACRT including staff supervision & performance and chairing goal planning meetings.

Main duties of the job

Main duties include:

  • Act as an expert Allied Health Professional in the field of community rehabilitation
  • Chair multi-disciplinary meetings
  • Provide expert training, advice and education to Allied Health Professionals and other health, social & voluntary care partners
  • Communicate effectively with clients, carers, colleagues and partners using a range of verbal and non- verbal communication strategies/tools
  • Provide daily operational and clinical leadership to ensure smooth running of the Adult Community Rehabilitation Team
  • Provide expert supervision for members of the multi-disciplinary team
  • Collaborate with colleagues to develop partnership working both within the organisation and across neighbourhoods and other organisational boundaries

    Knowledge, Training and Experience

    Essential criteria

  • Degree OT, PT, SLT, Dietitian incl HCPC registration
  • Current issues in the delivery of community rehabilitation
  • Significant experience of providing clinical or professional supervision to practitioners from own discipline
  • Proven experience of working with clients with physical or neurological impairment
