Main duties of the job

A vacancy has arisen for a permanent full time band 6 District Nurse Caseload Holder working within the Arfon District Nursing Service.

The Post holder must hold or be in working towards Community Specialist Practitioner qualification and be a Registered Nurse

The post holder will work jointly with community nurses who are based within the Arfon District Nursing team.

Qualifications/and or knowledge

Essential criteria

  • NMC Registered first level nurse
  • District Nurse Specialist Practice Qualification / Certificate, Diploma, Degree or working towards
  • Nurse prescribing qualification or working towards
  • Evidence of ongoing education and professional development.
  • Mentorship and assessment training.
  • Desirable criteria

  • Management qualification e.g. ILM, RCN Leadership
  • Experience

    Essential criteria

  • Community experience as Band 5 or equivalent
  • Experience of working with patients who have acute / complex needs
  • Varied clinical experience and multi disciplinary working.

  • Ability to assess, plan, implement, evaluate and delegate appropriately the delivery of care.

  • Ability to work collaboratively with members of the Primary Health Care Team and other agencies.

  • Awareness of the changing health agenda, which affects community practice.

  • Knowledge and evidence of application of consent.

  • Demonstrate timely travel arrangements between patients homes, clinics and surgeries.
