Main duties of the job

The role of the Cancer Tracker/Clerk is to take proactive steps to ensure progression of the cancer patient journey from point of suspicion of cancer through the diagnostic pathway to diagnosis and treatment or to the point where the patient is no longer deemed suspicious of having cancer (i.e., downgraded) in accordance with Welsh Government target guidelines and requirements. The post holder will work closely with the Cancer MDT Coordinator/Trackers and Cancer Services Management Team, liaising with Administrative and Clerical Staff to progress pathway and ensure timeliness. To identify and highlight constraints to Cancer Services staff, including liaison with the departments involved in providing care and core services, ensuring that planned steps in the diagnostic pathway progress smoothly and in a timely manner with the overall purpose of ensuring timely diagnosis to assist with achieving Welsh Government cancer targets and positive patient outcomes and survival.

Dealing with general enquiries to Cancer Services Office, entering data onto the cancer database and providing general office duties and support to the Cancer Coordinators, Cancer Team Leader and Cancer Services Manager.

Detailed job description and main responsibilities

  • Tracking of suspected cancer /cancer patients which will require communication with other departments within the organisation and external to the organisation to enquire about timely next steps in the patient pathway and to escalate next step dates where necessary (patient contact is minimal)

  • To source, collect and validate accurate data from differing PAS systems, in accordance with agreed practices and standards;
  • To ensure cancer pathway data is recorded and pathway issues are reported to the departmental management team in a timely manner
  • To actively support the Cancer Coordinators/Trackers ensuring patients are not delayed to a cancer diagnosis and ensuring suspected cancer patients are downgraded in a timely manner once the patient is deemed no longer suspicious of having cancer
  • To support the Secretarial staff in providing advice and guidance on Cancer Pathway related matters
  • To validate data to diagnosis or downgrade as instructed by cancer management team
  • To collect additional data on an ad hoc basis when required by line manager
  • To produce statistical information in report format using Excel
  • To develop and maintain effective working relationships with other staff in departments across the Health Boards sites

  • Communicate on a daily basis with administration staff and Cancer Services team members (within and outside of the organisation) for tracking of cancer patient pathway purposes
  • Recording accurate data regarding patient pathways on a daily basis using keyboard skills and a number of IT systems maintaining patient pathway records from point of suspicion to diagnosis, to ensure information is accurate and up to date for future reference, complying with the Data Protection Act

    Qualifications & Knowledge

    Essential criteria

  • Good standard of general education
  • IT skills
  • Previous knowledge and experience of WPAS and other databases, software systems used in the NHS and office environment experience essential e.g. word, email, RADIS.
  • NVQ Level 3 or equivalent experience.
  • RSA Level 2 or equivalent experience.
  • Desirable criteria

  • Medical terminology knowledge
  • Understanding of the function of MDT and the cancer pathways.
