Main duties of the job

To abstract, analyse, translate and data enter complex medical patient care records into alphanumeric codes using the ICD-10 and OPCS 4 classifications in an accurate and timely manner.

Use judgment and knowledge of coding procedures and conventions to translate information where diagnosis/procedure is not transferable to exact code.

Detailed job description and main responsibilities

Act as a mentor and assist in the education of less experienced and trainee clinical coders.

Regularly liaises with clinicians to maintain accurate clinical coding.

Ensure accuracy and completeness of clinical coding.

Locate and collate missing source material e.g. missing case notes, histology results.

Communication

  • To liaise with clinical, nursing, secretarial/clerical and portering staff to facilitate timely and accurate receipt of clinical source documentation for coding.
  • Where appropriate, visit the wards to obtain information for coding.
  • Communicate and explain complex coding rules to medical, clinical and administrative staff.
  • When necessary locate patients medical records to obtain source documentation on incomplete episodes.
  • To contact and liaise with community hospitals, as necessary, to obtain complex clinical information where medical records have been sent out with patients.
  • Contribute to team meetings on specific clinical coding queries, coding audit feedback and workshops attended.

  • GCSE/Equivalent education including English & Biology.
  • To work in the UK, all candidates who are not UK or Republic of Ireland (RoI) nationals require sponsorship to obtain either a Health and Care visa or Skilled Worker visa , unless you have permission to work via another route.
