A rare opportunity has arisen in the Community High Weald, Lewes and Havens Parkinson’s Disease specialist team

We are looking for a Band 7 Parkinson’s Disease Specialist Nurse to work as part of the team providing care, support and advice to people affected by Parkinson’s and their families. This is a varied role working in the community undertaking home visits, as well as seeing patients and their families within the clinic environment

This post is part time 30 hours over 4 days. With support and supervision from the consultants and other PD specialist nurses, the post holder will develop an in-depth knowledge of PD, refine their clinical skills to manage the care of patients across the disease spectrum, plus, develop knowledge in educating patients, carers and healthcare professionals to manage Parkinson’s Disease. Training and development opportunities will be offered

Applicants should be dynamic, motivated and keen to develop in this role. Strong interpersonal skills are essential to work collaboratively with people with Parkinson’s, their carer’s and the wider clinical multidisciplinary team.

This post would suit an already established Parkinson’s Nurse or a Community Staff Nurse wishing to develop their knowledge and skills in Parkinson’s disease.

Car driver / access to a car is essential for the role

Main duties of the job

1. Perform assessments of patients with Parkinson’s disease utilising specialist diagnostic procedures and equipment to formulate diagnoses and implement specialist treatment plans

2. Liaise daily with primary/secondary/community and other agencies involved in the management of patients with specialist needs

3. To manage caseloads and staff working in a specialist role ensuring a timely response to patients and colleagues/partners

4. To ensure that the post holder and the specialist team are working to agreed service specifications and specialist standards of care

5. To provide timely specialist advice and support to patients, their relatives and carer’s, other SCFT staff and our colleagues and partners in primary/secondary and third sector organisations

6. To assist in the design, delivery and evaluation of education and training programmes delivered to patients, staff and partners

7. To participate in team development plans including annual appraisals

8. To work as a specialist member of the MDT working with other community teams ensuring specialist clinical support is accessible and responsive to patients, their carer’s and colleagues

9. Promote, monitor and maintain best practice standards and health, safety and security within the specialist service
