Main duties of the job
This post is for a part time Young Onset Dementia Nurse.
The successful candidate whilst working closely with a multidisciplinary team will receive referrals directly from the GP’s and provide short interventions for individuals with Dementia. You will sign post service users to other statutory agencies and a range of third sector organisations.
The successful candidate will be expected to undertake assessments, risk assessments and risk management plans. There is also the opportunity to co-facilitate therapeutic groups. We are looking for a good team player who has excellent organisational skills and the ability to prioritise their time effectively.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
