Main duties of the job

Act as first point of contact for all referrals/enquiries to the CHC Team
 Ensure that all communications with members of the public, staff and any 
other agency are handled confidentially and professionally, in order to aid the 
delivery of a high quality service.
 Demonstrate excellent and advanced communication skills on confidential 
issues such as disputes or complaints when dealing with telephone 
calls/correspondence from members of the public, Solicitors and MPs, Health 
Professionals, Senior Managers both within the NHS, Local Authorities and 
non NHS organizations and in the absence of CHC Managers/Matron refer to 
appropriate person/department.
Take telephone messages in the absence of the CHC Team and if necessary 
signpost the caller to the correct person who can help with their query.
 Monitor and ensure correct panel outcomes, information is passed to finance 
department via liaising with nursing homes/monthly returns regarding any 
change in funding/accommodation etc (Deceased, New Patients, 
amendments).

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply

Deal with multi faceted information with regards to CHC i.e patients sensitive 
information with issues of confidentiality in the appropriate way.
 Produce reports for the team and CHC Team Managers when required.
 Establish and maintain efficient filling system for key documents and 
correspondences both electronically and manually.
 Manage the departmental BCU application mailbox and carry out appropriate 
action.
 To maintain and manage electronic diaries for CHC Modern Matrons and Team 
Manager and Nurse Reviewers.
 Ensure that letters relating to ratified, deferred or disputed CHC applications are completed and sent out to relevant parties. 
 
Arrange nursing reviews/ Multidisciplinary Meetings on behalf of the CHC Team and send out nursing review letter to all appropriate persons.
