Main duties of the job
The post holder will assist in the implementation of the National Framework for Continuing Health care (2022) and ensure that appropriate monitoring review procedures are implemented and maintained as set out in Government guidelines. The post holder will play a supporting role in providing clinical advice in all aspects of the Continuing NHS Care process to a locality team.
The post holder is required to act autonomously throughout the assessment process to ensure that healthcare funding policies are adhered to in accordance with the Welsh Assembly Government Framework, using analytical and judgemental skills to interpret these for each individual case.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Detailed job description and main responsibilities
Ensure that the assessment functions around healthcare applications are completed in a robust and timely manner. Obtain information from a variety of sources to ensure the timely processing of healthcare funding applications and assessments.
To work within the Multi-Disciplinary Care Team giving specialist advice and providing clinical supervision with regards to applications for Continuing health Care.Â
To monitor the quality and consistency of care provided to individual service users in care homes (Nursing) and reporting any concerns to LeadNurse/Continuing Health Care Manager
