Duties and Responsibilities

  • To work together with, and compliment, the existing multi-agency community care team, in extending the available care to patients and their families/carers, who are in the last weeks of life at home?
  • To work autonomously and unsupervised in the community, under the direction of the Hospice at Home Team Leader.
  • To assess the individual needs of the patient and their families / carers.
  • To provide basic nursing care to patients, e.g. personal care, catheter care, minor dressings, stoma care, and offer respite support to their families / carers, according to QCF / NVQ competencies.
  • To provide social care to patients, e.g. this may include minimal meal preparation and nutritional support.
  • To provide personal care to deceased patients as appropriate and according to the wishes of the deceaseds family / carers.
  • To provide holistic care to patients and their families / carers, being ready to meet searching questions on a personal level, demonstrating effective communication skills.

    This post is fixed term/secondment for 12 months to meet the needs of the service. If you are interested in applying for the secondment position, you must obtain permission from your current line manager prior to applying for this post.

    Clinical Practice

  • To demonstrate adaptability and responsibility when dealing with unpredictable situations, e.g. support to distressed patients and their families / carers and contacting appropriate healthcare professionals / agencies in emergency situations.
  • To undertake other specialised procedures, according to the patients need, appropriateness and the health care support workers competencies. For all procedures, health care support workers need to be resourceful within the constraints of the patients home environment.
  • To be responsible and accountable for performance and standard of care given.
  • To be responsible for organising tasks in a flexible manner according to priority of need.
  • To communicate relevant information to the rest of the team concerning patient and their families/carers and respect confidentiality when necessary.
  • To document relevant information in the patients care plan.
  • To participate in family meetings with the multi-disciplinary team when appropriate.
  • To offer support where appropriate to bereaved relatives.
  • To participate in clinical audits as required.
  • Networking and communications 

  • To interact effectively and develop positive relationships with service users.
  • To report to trained nurses, GP and colleagues in other service areas and localities, as appropriate, any changes in conditions or behaviour of patients by communicating observations and factual information.
  • To communicate effectively with the Hospice at Home Team Leader / District Nurse for advice and support regarding care plan / integrated care pathway issues or other operational aspects.
  • To participate in team discussion groups / meetings in a positive and constructive way.
  • To negotiate all off-duty, annual leave and study leave with the Hospice at Home Team Leader.
  • Professional Issues 

  • To comply with Health and Safety regulations and assist in maintaining a safe working environment.
  • To adhere to relevant Departmental and Trust Policies so that the integrity of the service is maintained.
  • Professional Development 

  • To participate in reflective practice/supervision.
  • To undertake relevant departmental training in relation to caring for the dying.
  • To continue with personal development, QCF training and education through attendance at seminars, courses and mandatory training.
  • To contribute positively in receiving the Personal Development Review with the Hospice at Home Team Leader.
  • Change Management 

  • To positively support specialist palliative care clinical leads and managers in the process of change and improvement.
  • To support the management process of strategic planning and review, to enable improvements in local services.

    Qualifications

    Essential criteria

  • QCF/ NVQ in Health Care Level II Qualification or equivalent
  • Willingness to undertake QCF in Health Care Level III Programme
  • Desirable criteria

  • Experience of working in a primary care setting
