It’s an exciting time for our Urgent Community Response (UCR) Service as we expand into the High Weald, Lewes, and the Havens (HWLH) area. We are recruiting to our new teams who will work with patients in their homes across the HWLH communities.
As part of our expansion, we are looking for Occupational Therapists to join us, so if you’re a motivated and enthusiastic occupational therapist looking to be part of a supportive team and use your skills to provide excellent acute community care, we’d love to hear from you.
This is an amazing opportunity to become a member of the team as an occupational therapist working together to help patients stay in their homes, making sure they avoid unnecessary hospital admissions and supporting them through the first few critical days after they leave hospital. The focus of this role is delivering essential holistic assessment and treatment to patients with acute, diverse needs across the area.
Main duties of the job
In this role you will work with patients within their home providing acute clinical assessment, planning treatment and intervention for patients with a variety of conditions, identifying ongoing needs and appropriately referring to other longer-term services within the area. You will be a valued and supported member of the team, contributing to team decisions and partaking in regular in-service training.
If you are new to Sussex Community NHS Foundation Trust, you will be given the opportunity our preceptorship programme, through protected time and regular clinical supervision with dedicated registered professional support. You will also join the Band 5 Rotation, developing your skills across different clinical areas.
You will need to able to drive with a full UK licence due to the geographic area covered by the team.

We are a multidisciplinary team working flexibly to provide a seven-day service, 365 days a year, between 8am and 8pm. We deliver:
The team works closely with other services within Sussex Community NHS Foundation Trust, as well as our acute and community partners, GPs, South East Coast Ambulance Service (SECAmb), adult social care and voluntary and third sector organisations.
