Main duties of the job
This new role will provide an evidence based Quality Improvement focus within Area Community Nursing Services, autonomously leading on Quality Improvement methods to support sustainable growth in the localities.
The post holder will be identified as a clinical expert and have a high clinical profile ensuring provision of effective and efficient care within Area Community Nursing Services. The post holder will manage and co-ordinate the delivery of a strong and robust training programme both formally and informally to Community Nurses across Gwynedd and Mon regarding pressure ulcer prevention, wound management, Leg Ulcer management as well as the effective and timely use of pressure relieving equipment in the community, acting as an expert resource for patients, carers, nursing staff and other colleagues in regards to pressure ulcer and leg ulcer/chronic oedema prevention.
The post holder will work autonomously as clinical quality improvement lead for West Area to ensure that patient outcomes are improved by timeliness and consistency of care in relation to Tissue Viability issues, pressure ulcer prevention and Leg ulcer management as well as improving skills and knowledge base within the Community Nursing Service. Leading on the cost effective procurement of specialist products/equipment as well as responsibility for the management and monitoring of budgets for pressure relieving equipment resources within area of responsibility.
