Main duties of the job

  • Provide support and offer advice to community and ward staff on completion of referrals to the Community Resource Team (CRT)
  • Receive patient referrals from SPOA and Acute Wards, GPs and Community Nurses by either telephone, fax or e-mail.
  • Scrutinise all patient referrals liaising with staff to provide any outstanding or additional information to ensure the patient flow team and community staff receive accurate assessments.
  • Accurately record messages received to the service and ensure they are passed on to the appropriate person in a timely manner
  • Access patient information using WPAS, PARIS, Therapy Manager, Malinko, Myddin, IFIT, to check patient information, e.g. inpatient history and discharge/transfer dates, respecting patient confidentiality at all times
  • Collate information on referrals to CRT and record for the teams as required.
  • Book appointments for CRT patients on Malinko, Myrddin, WPAS, PARIS or Therapy Manager systems
  • Work closely with the Admission, Discharge and Transfer Team to ensure direct referrals from A&E/AMU have received an appropriate assessment and are suitable to receive care in a Community Hospital setting or Enhanced Care Service (ECS). 
  • The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
