Main duties of the job
The successful candidate will have the skills to deliver holistic, safe, effective care to patients within the community and hold a complex caseload of individuals aged 18-65 with severe and enduring mental health issues and will work collaboratively to assess the needs and create care & treatment plans to meet the identified needs and risks identified. The post holder will be required to work independently and liaise with a variety of agencies and third sector organisations plus the individuals carers and family members. This post will be initially located within the Community mental health team in Conwy however there will be a requirement to work within other teams and this may include inpatient settings in the future.
You will have exceptional communication skills and the ability to build and maintain effective working relationships, demonstrating a can do attitude.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
