We are looking for experienced support workers for full time rotas within an Intermediate Care Centre, assisting people over the age of 18 who require rehabilitation, reablement, convalescence and ongoing assessments following a physical illness or trauma.
You will help determine the best outcomes for customers, involved in ensuring they are included in all aspects of their lives and decisions. Every day will be different, so you can expect to undertake work that is varied and rewarding. You will work very closely with health professionals to support people to progress and enable them to reach their potential.
