The post holder should have a knowledge and understanding of The Discharge Process, Funded Nursing Care and Continuing Healthcare. You should also have excellent communication, assessment and documentation skills and be committed to working within a multidisciplinary environment.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Qualifications

Essential criteria

  • Registered Nurse registered with the NMC.
  • Relevant evidence of post qualification study
  • Professional knowledge acquired through relevant evidence of post qualification study
  • Desirable criteria

  • Masters in Nursing or working towards Master degree or equivalent.
  • Experience

    Essential criteria

  • Substantial post registration experience of working in older persons and/or working age adult clinical specialities with a working knowledge of both in hospital and community settings
  • Demonstrate significant experience of working at Band 5 or above within adult services acute/ community hospital.
  • Desirable criteria

  • Experience of service development.
  • Change management experience.
  • Provide specialist training to professionals, relatives and patients.
  • Experience of organising transfers of care to longer term environments, and liaising with health commissioners to broker accurate and effective packages of care.
  • Knowledge

    Essential criteria

  • Specialist knowledge underpinned by evidence based theory and clinical tools
  • Advanced understanding and knowledge of issues related to health and social care needs, placement issues, homelessness needs and an awareness of the interface with general health needs.
  • Specialist knowledge of risk assessment and management when planning a safe patient discharge.
  • Ability to undertake teaching and training of staff
  • Knowledge of the Mental Capacity Act 2005 and the Mental Health Act 1983, DOLs, MAPPA, MARAC, POVA. Mental Health Measure and child safeguarding. The ability to communicate this to all DGH staff.
  • Specialist knowledge of community based services to ensure appropriate signposting to other agencies.

  • Advanced knowledge of older persons mental health assessment tools, and the ability to use and select these without direction.
  • Motivational Interviewing skills.
  • WARRN (risk management) training.
  • Ability to speak Welsh.
  • Knowledge of a wide range of complex physical health problems which impact upon mental health needs.

  • The post holder will initiate advocacy arrangements when required.
  • Ability to communicate highly complex and sensitive information to patients, carers and professionals.
  • Deliver and provide specialist advice and training.
  • Ability to build up effective working relationships across Divisions outside of Adult Health, developing networks to improve the patient experience
