Main duties of the job

The post holder will support the Senior Stroke Coordinator with the leadership and management of the provision of care within the ward providing a visible leadership presence. You will be responsible for ensuring that high standards of clinical evidence based care are continuously achieved and monitored, ensuring a positive learning environment. The successful candidate will possess excellent clinical skills in relation to all aspects of care, combined with a caring attitude and good interpersonal and communication skills with patients and all members of the multidisciplinary team.

JOB PURPOSE

In conjunction with the Senior Stroke Coordinator, co-ordinate the provision of a specialist stroke service to patients referred to the service.

In conjunction with the Senior Stroke Coordinator, facilitate and coordinate the patient journey through from admission to discharge into primary care, identifying and monitoring the long term needs of stroke patients through close liaison with the primary care health teams.

In conjunction with the Senior Stroke Coordinator, coordinate delivery of care for stroke patients within a clinical governance framework adhering to national and local standards.

In conjunction with the Senior Stroke Coordinator, ensure that the requirements of the Sentinel Stroke National Audit Programme are met.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Essential criteria

  • Educated to Degree Level or working towards achieving degree.
  • Evidence of post graduate training relevant to care of patients with a diagnosis of stroke.
  • Willingness to work towards achieving Masters qualification relevant to area of work.
  • Registered with Nursing and Midwifery Council.
  • Desirable criteria

  • Experience

    Essential criteria

  • Post registration experience at a minimum of Band 5 for 2 years in care of patients with a diagnosis of stroke. This experience must have been within the last four years.
  • Experience in monitoring and evaluating the provision of stroke treatment and care
  • Experience of working across organisational boundaries in a health care setting
  • Experience of working with an MDT across acute, community and primary care

  • Excellent communication skills both written and verbal when interacting with members of the team, other professions, clients and their families, demonstrating the ability to impart complex information to all of the above recognising different levels of understanding
