Main duties of the job

The Band 6 caseload holder within the service will have direct oversight of a group of patients who are supported through the function of a virtual ward. This will include patients in the ED/Frailty Area awaiting transfer home, in the home environment or in a step up/down residential bed in the community.

 This is a full time post, covering both early and late shifts and will include weekend working.

Full induction into this post will be supported, with opportunities for further career development.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Essential criteria

  • NMC Registered first level nurse
  • Evidence of ongoing education and professional development to Diploma Level or equivalent level of experience
  • Mentorship & Assessment training

  • Management qualification e.g. ILM, RCN Leadership
  • Independent Prescribing
  • District Nurse Specialist qualification or equivalent community nursing / development qualification

  • Significant community experience as a Band 5

  • Varied clinical experience and multi disciplinary working
