Diabetes Care For You is a pioneering and innovative multi-professional community based diabetes service, consisting of Consultants, Nurse Consultant, Diabetes Specialist Nurses, Dietitians, Podiatrists and Psychotherapists.

This is an exciting opportunity for an experienced diabetes specialist nurse with drive and ambition to have a clinical leadership and expert role. The team were finalists in the 2020 HSJ Value Awards and 2019 winners of a Quality in Care award, in the Mind and Body Health Together category.

Main duties of the job

The post holder will be expected to provide and lead on the delivery of high quality specialist integrated care for people with diabetes at all stages of the care pathway. You will act as a resource for other healthcare professionals and participate in nurse led clinics across Brighton & Hove and High Weald, Lewes & Havens

  • Work autonomously and carry a caseload of patients who require senior specialist intervention.
  • To provide contemporaneous records and reports regarding patient’s ongoing care and management consistent with legislation, policies and procedures
  • To assist with supervision, recruitment, induction and appraisal of junior staff
  • To facilitate education programmes tailored to the needs of patients with diabetes, underpinned best practice and using a patient centred approach
  • To support the efficient delivery of the diabetes service across both Brighton and Hove and High Weald Lewes and Havens locality
  • To assist in the provision of the service in accordance with the wishes of the commissioners.
  • Delegate work appropriately to other clinical staff
  • To support personal development planning and objective setting and review. To assist with performance management issues as required.
  • To maintain competence in specialist area of practice by means of peer review, clinical supervision, journal and other reading and attendance on courses and conferences.
  • Prescribe and review medication (if appropriate prescribing qualification has been obtained and governance in place) for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national practice protocols, and within scope of practice.
  • To participate in team meetings and update staff on operational matters, policy updates and training requirements.
  • Review departmental and corporate policies to update in line with new legislation and actively assist in development of policies that directly involve diabetes.
  • To assist participation of department in audit in collaboration with the departmental audit lead.
  • To assist professional manager and patient safety lead in matters pertaining to patient safety and risk management.
  • To assist in dealing with complaints to the service in association with the Customer Service Team.
  • To undertake any other duties that may be required which are consistent with the grade and responsibility of the post.
  • To provide clinical leadership and vision to support the Nurse Consultant and diabetes specialist nurse roles, influencing the development of an integrated community diabetes pathway.

  • To support the future planning of service developments in relation to community and nursing led care in diabetes alongside the Nurse Consultant.

    Qualifications
    Essential criteria

  • RGN first level nurse
  • Post graduate level degree or equivalent
  • Advanced Physical Assessment course Masters Module/accredited physical assessment course
  • Independent prescribing
  • Advanced Nurse Practitioner Masters qualification or evidence of study at masters level in a related subject
  • Mentoring course or equivalent
  • Experience
    Essential criteria

  • Extensive clinical experience five years in Diabetes management
  • Critical care/emergency care or primary care first contact experience at band 7 level
  • Evidence of up-to-date based knowledge and skills in managing patients with diabetes
  • Making differential diagnosis
  • Evidence of ability to maintain and monitor high standards of care
  • Evidence of continuing professional development
  • Desirable criteria

  • Experience of human resource management including performance management, managing sickness and absence, recruitment and retention of staff
  • Experience of reporting against targets and risk management

    Essential criteria

  • Advanced physical assessment and history taking
  • Experience of using clinical software pro-grammes eg SystmOne, PAS, PACS
  • Ability to empower patients by appropriate health teaching and advice
  • Knowledge of health and safety within clinical practice and associated areas
  • Knowledge of local and national policies relevant to Diabetes
  • Experience of clinical audit
  • Knowledge of clinical standards, i.e. IC, CQC and evidence of implementing and monitoring these in practice
  • Able to recognise clinically when a patient required referral in both the emergency and planned situation
  • Experience with the development of proto-cols, guidelines, policies and procedures
  • Evidence of ability to direct and co-ordinate programmes of care working autonomously and collaboratively
