You will be working within the personalised care team for the Valleys Primary Network (PCN). The Valleys PCN covers the semi-rural populations across the Holme and Colne Valley and has six member practices, who work together to improve the healthcare of the local population together with local partners within the NHS, local authority and voluntary sector.

They will work alongside people to coach and motivate them through multiple sessions, supporting them to identify their needs, set goals, and help them to implement their personalised health and care plan.

Willingness to work flexible hours, including evenings and weekends on occasion.

Travel is an essential part of this job and therefore it would be extremely difficult for you to do your job effectively if you did not use a car for all or most of the time. It is essential that you hold a full and valid driving license and it is expected that you will either use a council vehicle or your own car (exceptions can be made for disabled applicants).
