Main duties of the job

The successful candidate will be responsible for the ongoing Nursing Assessments of patients to facilitate discharge from the hospital through the Discharge to Assess model of care. The successful post holder will work within a supportive multi-disciplinary team to promote the Home First ethos through ongoing therapeutic interventions in the patients own environment. This is an exciting opportunity to be part of a developing service within BCUHB East linking in with the existing Community Resources Team. You will be actively involved in the future development of the service, if you looking to be part of an exciting new service please do not hesitate to get in touch.

  • Evidence of nursing related qualification to diploma or degree level

    Essential criteria

  • Post holder would need specialist experience in discharge planning, continuing health care eligibility criteria, and the legal aspects of discharge, Discharge to Recover and Assess and discharge planning.
